I Need a Provider

Providers of services to people with developmental disabilities may be independent (self employed individuals) or agencies. In either situation, providers must be certified through the Ohio Department of Developmental Disabilities in order to provide services through waiver funding.

Independent Providers

An independent provider is a self employed person who provides services to people with developmental disabilities.

Agency Providers

An agency provider is an entity that directly employs a chief executive officer (CEO) and at least one other person for the purpose of providing services to people with developmental disabilities.

Day Service Providers

Adult day services are available for adults enrolled on Individual Options and Level One Medicaid waivers administered by the Ohio Department of Developmental Disabilities (DODD). The services include Adult Day Support, Vocational Habilitation, Supported Employment, and Non Medical Transportation.
