POSITION: Eligibility Coordinator – Responsible for coordinating and conducting comprehensive eligibility determinations for individuals seeking services though the Shelby County Board of DD.
QUALIFICATIONS: Minimum of a Bachelor’s Degree required – preferably in a human services related field. Ability to obtain and maintain SSA Certification and Children’s/Ohio Eligibility Determination Instrument (C/OEDI) Certification (The Board of DD will provide guidance for the certification process if needed.) Experience performing formal assessments and working directly with people with developmental disabilities preferred. Excellent written and oral communication skills. Ability to transport individuals with a valid driver’s license and satisfactory driving record. Successful completion of a BCI/FBI background check and multiple database background checks as defined by Rule 5123-2-02.
DUTIES: General duties include, but are not limited to:
- Coordinates and conducts eligibility determinations using established criteria and tools (such as the Ohio Eligibility Determination Instrument – OEDI or Children’s Ohio Eligibility Determination Instrument – COEDI).
- Collects and reviews medical, psychological, educational, or other necessary documentation to assess eligibility.
- Collaborates with families, medical professionals, school personnel, and other community partners to gather necessary documentation and support information.
- Communicates eligibility decisions clearly and compassionately to individuals and families.
- Maintains necessary tracking systems including eligibility redetermination.
- Maintains accurate and confidential records in accordance with Ohio Department of DD regulations.
- Serves as a knowledgeable resource for questions related to eligibility processes and requirements.
- Completes the Pre-Admission Screening and Resident Review (PASSR) process.
- Completes current and immediate need waiting list assessments in accordance with Ohio Administrative Code.
- Assists with system enrollment as needed.
- Administers Extraordinary Care Instruments to determine needed support levels required.
- Responsible for working with the Director to develop policies and procedures related to the intake process.
- Promotes positive relationships with families, schools and other agencies and the community.
- Provides information to schools and other agencies about the intake process for DD services.
- Refers individuals to other agencies as needed, throughout the intake/eligibility process.
- Responsible for Medicaid Billing and documentation as appropriate.
COMPENSATION: Annual salary starting between $46,640 and $69,960 based on relevant experience. Benefits include Ohio Public Employees Retirement System (OPERS); medical, dental, vision, life insurance; paid sick, vacation and personal leave. Eleven paid Federal Holidays and extended holiday periods in July and December.
HOURS: 40 hours per week, generally 8:00am to 4:00pm. Schedule will sometimes require flexibility to meet the needs of the individuals we serve. Remote work opportunities available after initial training period completed.
