POSITION:                  Resource Coordinator   

QUALIFICATIONS:     Bachelor’s Degree required, preferably in human services, education, social work, rehabilitation or other closely related field. Ability to work effectively with minimal supervision. Must have availability outside of routine business hours, including nights and weekends. Ability to obtain and maintain Service and Support Administration (SSA) certification from the Ohio Department Developmental Disabilities. Excellent written and oral communication skills. Successful completion of a BCI/FBI background check and multiple database background checks as defined by Rule 5123-2-02.

DUTIES:   General duties include, but are not limited to:

  • Provide expertise and oversight of the entire environmental accessibility adaptions, specialized medical equipment and supplies, assistive technology, and remote support process, from referral through coordinating an assessment, working with vendors for equipment or contractors for modifications, to completion.
  • Build relationships with current service providers, vendors and contractors and recruit new resources to build capacity.
  • Organize and track all environmental accessibility adaptions, specialized medical equipment and supplies, assistive technology, and remote support service referrals, correspondence, reports, invoices, funding requests, verification of waiver service documentation, and payments.
  • Assist teams to troubleshoot concerns with MUI’s/UI’s, toileting, transfers, changing, dressing, personal care, lifting, employment, safety, mobility, or other needs and secure resources to assess and meet identified needs.
  • Assist with housing searches. Assist with hospital/facility discharge planning and conducting residential searches for individuals and families. Maintain list of residential placements and funds available including grants, rental assistance funds, capital grants and other funding sources.
  • Develop relationships with community landlords, Shelby County Metropolitan Housing and other residential support providers to assist with housing searches for families and individuals.
  • Organize and coordinate the FANs network and facilitate all service requests from start to finish.
  • Maintain lists and contacts of available community resources in order to connect Service and Support Administrators (SSAs) or Service Coordinators to resources such as food pantries, shelters and other assistance resources.
  • Develop and maintain contacts and contracts with therapists and certified staff to provide recommendations for adaptive equipment or home modifications.
  • Stay up to date on rules, regulations and maintain local contacts for trouble-shooting with other organizations such as Medicaid, Social Security, etc.
  • Assist county board staff with locating resources in crisis situations that may arise with individuals served or families. Coordinate training for individuals, families, guardians, and providers on environmental accessibility adaptions, specialized medical equipment and supplies, assistive technology, and remote support services as needed.
  • Attend trainings and participate in educational opportunities regarding environmental accessibility adaptions, specialized medical equipment and supplies, assistive technology, and remote support services, share information learned and connect vendors directly to the SSA Department.
  • Monitor and maintain all ceiling lift systems installed for all individuals using them, ensure yearly inspections are completed and coordinate any maintenance or repairs needed.
  • Coordinate any maintenance, repairs or upgrades needed to environmental accessibility adaptions, specialized medical equipment and supplies, assistive technology, and remote support services equipment as needed.
  • Maintain inventory of specialized medical equipment and supplies stored and disburse as needed. Work with local medical supply donation agencies to obtain needed equipment or donate equipment that may not be needed.
  • Schedule, track and attend all appointments as needed, coordinate assessments, contractor work, deliveries and set up with internal staff, vendors, contractors, individuals, families, guardians, and providers.
  • May provide support to Champaign County as requested. No more than 10%.

COMPENSATION:  Pay starting at $21.31 per hour, or more based on relevant experience. Benefits include Ohio Public Employees Retirement System (OPERS); medical, dental, vision, life insurance; paid sick, vacation and personal leave. Eleven paid Federal Holidays and extended holiday periods in July and December.

HOURS:   40 flexible hours per week, generally 8:00am to 4:00pm. Schedule will require flexibility to meet the needs of the individuals served. Remote work opportunities available after initial training period completed.
