I Need a Provider
Providers of services to people with developmental disabilities may be independent (self employed individuals) or agencies. In either situation, providers must be certified through the Ohio Department of Developmental Disabilities in order to provide services through waiver funding. The Shelby County certified list of providers is below.
An independent provider is a self employed person who provides services to people with developmental disabilities.
An agency provider is an entity that directly employs a chief executive officer (CEO) and at least one other person for the purpose of providing services to people with developmental disabilities.
